- National visa application form - Filled in and signed
- Photographs - 2 identical recent passport-size photos (1 attached to the form)
- Passport + copy - Valid at least 3 months beyond intended stay; copy of data page
- Travel insurance - Covering medical expenses, urgent care and repatriation
- Police clearance certificate - Issued less than 90 days before applying; apostilled by MOFA Pakistan
- Employment contract / job promise - Or individual expression of interest from the employer
- Proof of means of subsistence - From the contract, or employer's term of responsibility with recognised signature
- Professional qualification proof - Only if the profession is regulated in Portugal
- Birth certificate - Apostilled; issued within the last 6 months
- Family Registration Certificate (FRC) - Apostilled; issued within the last 6 months
- Marriage / divorce certificate (if applicable) - Apostilled; issued within the last 6 months
