- e-Konsulat visa form - Printed and signed
- Colour photo - 3.5 x 4.5 cm, white background, not older than 6 months
- Passport + copy of data page - Issued within 10 years, valid 3 months after return, 2 blank pages
- Travel medical insurance - Minimum EUR 30,000
- ID proving residence - Copy of your ID for the consular district
- Bank certificate - Last 6 months, stamped and signed by the bank branch
- Employer income letter - Income for the last 3 months
- Business owners: NTN, FBR tax certificate - Plus chamber membership certificate
- Hotel booking / travel plan - Bookings in every Schengen country visited
- Voivodeship-registered invitation - For private or sponsored visits, plus the sponsor's affidavit and bank statement
- Family Registration Certificate
- Flight reservation - Pakistan to Poland/Schengen and back
