- Passport + copy of data pageIssued within 10 years, valid 3 months after return, 2 blank pages
- Travel medical insuranceMinimum EUR 30,000
- ID proving residenceCopy of your ID for the consular district
- Bank certificateLast 6 months, stamped and signed by the bank branch
- Employer income letterIncome for the last 3 months
- Business owners: NTN, FBR tax certificatePlus chamber membership certificate
- Hotel booking / travel planBookings in every Schengen country visited
- Voivodeship-registered invitationFor private or sponsored visits, plus the sponsor's affidavit and bank statement
- Family Registration Certificate
- Flight reservationPakistan to Poland/Schengen and back
- Minors: notarised parental consentPlus parents' passports and birth certificate
