- Health declaration formEMGS form, scanned and uploaded
- Pre-VAL medical reportIn English, if not screened at an EMGS-registered clinic
- Personal bondSigned by your institution. Amount depends on your country
- eVAL (for the eVISA)Visa Approval Letter used for the MYVISA Visa With Reference application
